WHAT TO BRING

For your first visit, please arrive 15 minutes early to fill out paperwork, and please bring the following items:

• Insurance card (s)
• Referral from your primary care physician (if needed)
• Results of diagnostics testing (x-rays, scans, etc.)
• List of prescriptions and medical conditions
• Medical records

Our practice is affiliated with the following hospitals:

• Kent General Hospital
• Bayhealth Medical Center

From the South: Route 13 N into Dover, just past Rodney Village take the left exit onto South Governors Ave. Go 4 traffic lights and take a left on North Street. Go through 2 traffic lights, then over the train tracks, and turn left onto Banning Street, which is the entrance for Eden Hill Medical Center.

From the North: Route 13 S into Dover. Turn right onto Scarborough Road. Continue on McKee Road, which turns into Salsbury Road. Turn left onto North Street. Then turn right onto Banning Street, which is the entrance for Eden Hill Medical Center.

From the West: Route 8 into Dover and turn right onto Salsbury Road. Turn left onto North Street. Then turn right onto Banning Street, which is the entrance for Eden Hill Medical Center.
