WHAT TO BRING

For your first visit, please arrive 15 minutes early to fill out paperwork, and please bring the following items:

• Insurance card (s)
• Referral from your primary care physician (if needed)
• Results of diagnostics testing (x-rays, scans, etc.)
• List of prescriptions and medical conditions
• Medical records

• Bayhealth Medical Center

From the South: Route 13 N into Dover, just past Rodney Village take the left exit onto South Governors Ave. Go through 2 traffic lights and take a left at the First National bank of Wyoming onto Dover St. Go right at the stop sign onto S. New St. Urology Associates of Dover is the second building on the left, 740 S. New St.

From the North: Route 13 S into Dover. Continue just beyond the routes 13/113 split, turn right at the traffic light onto Court St. Pass the state Capital Building on the left and turn left at the first stop sign onto Lexington Ave. Turn right at the next stop sign onto Water St. Go through two traffic lights, turn left at the next intersection (S. New St). Go past Farmer's Market on the right. Just after the Farmers Market is our building, 740 S. New St.

From the West: Route 8 into Dover and turn right onto Lockerman St at the "Y." Turn right at the second light onto S. New St. Continue on S. New St. approximately 1/2 mile. Past the first traffic light you will pass the Farmer's Market on the right. Urology Associates of Dover is the third building past the market, 740 S. New St.
